1. Welcome
Affordable Care, LLC ("we," "us," "our," or "Affordable Care"), controls this website, www.affordabledentures.com, as well as the websites of our affiliated dental practices ("Site" or “Sites”), our text messages, our e-mail marketing campaigns, and other digital advertising (collectively, our "Services"). This Privacy Policy ("Privacy Policy") is applicable to you ("you," "your," or "user").
We have developed this Privacy Policy to inform you of our practices, as well as those of our affiliated dental practices located throughout the United States ("Affiliated Dental Practices"), regarding the collection, maintenance, storage, use, disclosure, and protection of your information. This Privacy Policy is effective when you use this Site, when you use our Services, when we contact you, or whenever you provide your information to us, or our Affiliated Dental Practices, including but not limited to, when you provide your information in-person, online, on the phone, in a text message, in an e-mail, through a written letter, or otherwise by some other means or manner ("Provide Information").
You should read this Privacy Policy carefully before you use our Services or before you Provide Information. Also, note that personal information collected by certain Affordable Care Services may have different privacy policies that apply. For example, personal health information (“PHI”) collected by our Affiliated Dental Practices may be covered under the respective Practices’ Health Insurance Portability and Accountability Act (“HIPAA”) Notice of Privacy Practices which can be found from the header or footer under Patient Support and clicking on "New Patient Forms" on any of our location websites located on affordabledentures.com Please refer to other privacy policies where applicable.
By accessing this Site, by using the Services, or by Providing Information, you accept and agree to be bound by this Privacy Policy, our "Terms of Use," found at https://www.affordabledentures.com/terms-of-use, and our HIPAA Notice of Privacy Practices for each of our Affiliated Dental Practices, which are herein incorporated by reference.
If you do not agree to be bound by this Privacy Policy, then do not use the Site or the Services, and do not Provide Information. Note that this Privacy Policy may change from time to time, and your continued use of the Services, after we have made changes to the terms, is considered acceptance of those changes. It is your responsibility to check this Privacy Policy for updates.

2. Information We Collect And How We Collect It
Affordable Care and our Affiliated Dental Practices obtain information about you in order to conduct business, to develop comprehensive statistics, to serve you better, and for other business-related purposes.
Information You Provide. We also collect all of the information that you provide to us through the Services and through any correspondence, whether through standard mail, e-mail, text message(s) or phone call(s). This information may include, but is not limited to, first and last names, demographic information, mailing address(es), e-mail address(es), phone number(s), credit card number(s), and any other information you provide to us. We may also collect information when you visit one of our Affiliated Dental Practices and provide information, including but not limited to, through interviews, through patient forms, and through other similar documentation and exchanges which may include information related to dental health. Note, we use processors to receive and process credit card numbers on our behalf. All of this is Personal Information, which can be used to personally identify you.
When you search for a location of one of our practices on a Site, we collect the information you provide, such as the city, zip code, and state. We use such information to improve search results and for our internal business-related purposes.
Third-Party Information. We also obtain information about our Users from third parties, including other businesses with whom we have advertising, professional, or other business relationships. Additionally, if you choose to access, visit, and/or use any third party social networking service(s) that may be integrated with our Service, we may receive your Personal Data and other information about you and your computer, mobile, or other device that you have made available to those social networking services, including information about your contacts on those services. For example, some social networking services allow you to push content from our Service to your contacts or to pull information about your contacts so you can connect with them on or through our Service. Some social networking services also will facilitate your registration for our Service or enhance or personalize your experience on our Service. Your decision to use a social networking service in connection with our Service is voluntary. However, you should make sure you are comfortable with the information your third-party social networking services may make available to our Service by visiting those services' privacy policies and/or modifying your privacy settings directly with those services.
Job Applicants. Job application information may be collected by us by email, or job application information may be collected on our behalf by our contracted third-party administrator(s) for applicant tracking purposes and by background check providers. These third-party administrators are permitted to process the information you provide only for purposes of providing services to us. We collect the information you provide to us, such as your name, email address, phone number, address, the content of any comments shared over email, your resume, and any cover letter you provide. For job applicants that are California residents, please see the Jurisdiction-Specific Notices section below for additional information.
Information We Automatically Collect. We and/or our third-party service providers collect information about users of this Site and our Services, including but not limited to internet connections, computer equipment, web browsers, websites visited before and after accessing the Site, IP addresses, and other similar information about traffic and usage.
We may also collect log data, which refers to certain information about how a user uses our Services or accesses our Site. Log data may include the pages or features of the Site that the user accessed, and the time spent on those pages or features, the frequency with which the Site is used, search terms, the links on our Site that a user clicked on or used, and related timestamp.
We also use these technologies to collect and store Internet or other electronic network activity information (such as your browser characteristics, device IDs and characteristics, operating system version, and language preferences) and information concerning your usage of our Site (including the link you used to reach a given webpage).
We use data collected automatically through the Sites for our business and commercial purposes, including to operate the Sites, provide access to certain portions of the Sites, and market our Services to you. You can set your browser to reject cookies, but that may limit your use of some convenient features of our Service(s) and Site and the Site may not function properly. The information we collect automatically may be aggregated with other Users' information. It may also be combined with other information that you provide directly to us, to our Affiliated Dental Practices, or that we receive from third parties. Moreover, the information we collect automatically may be de-identified by us and/or our service providers and we will maintain and use such de-identified data without attempting to re-identify the data.
With respect to any social media pages that we may use, please see the privacy policies and any cookie policies of the applicable social media providers as to their automated data collection practices.
The Site does not respond to Do Not Track signals or similar signals unless required by applicable law.
3. Third-Party Services Related To Information Collection
We use third-party service providers to help us operate our Services, who may collect, store, and/or process the information detailed herein. We allow access to our database by third parties that provide us with services, such as technical maintenance and market research, but only for the purpose of and to the extent necessary to provide those services.
There are also times when you provide information about yourself to us in areas of the Service(s) that may be managed or participated in by third parties. In such cases, the information may be used by us and by such third party(ies), each pursuant to its own policies. We do not control such websites and are not responsible for their contents or the privacy policies or other practices of such websites. Our inclusion of links to such websites does not imply any endorsement of the material on such websites or any association with their operators. This Privacy Policy only applies to our Site.
4. The Use Of Your Information
We and our Affiliated Dental Practices collect and process information about users in order to conduct our business operations as well as send automated messages regarding appointment dates/times, helpful follow-up treatment information and survey requests, and for the development of web statistics and user profiles. We make use of user information to enhance, improve, operate, maintain, and update the Services (including but not limited to, our programs and our systems) and to market our Services. We might also make use of your information to prevent fraudulent use of the Services, or to maintain a record of our relationship, interactions, and transactions with you.
We combine the information that we receive; for example, we combine information we receive online with information we receive offline; and we combine information we collect from you with information we receive from our Affiliated Dental Practices and from third parties.
The information you provide may be used to contact you from time to time, for example to notify you about functionality changes to the Services or terms changes to any of our policies, and to offer products or services that you may find helpful (provided that you have not opted out of receiving such notices). Your mobile opt-in data and any consent obtained for our SMS messaging program will not be shared, sold, rented, leased, or otherwise transferred to any third party or affiliate for marketing or promotional purposes. Mobile opt-in data is used only to operate our text messaging program with our messaging service provider. Unless you have specifically authorized us in writing to do so, we will never share your information, excluding mobile opt-in data and any consent obtained for our SMS messaging program, with third parties for their marketing, sales, or advertising purposes, nor will we use your information to market, sell, or advertise any third-party item or service where we receive financial remuneration in exchange for making the communication.
In addition to the purposes set forth in our HIPAA Notice of Privacy Practices, we may also use your information to:
- Deliver any products or services requested by you;
- Manage and attend to our relationship with you;
- Communicate with you;
- Measure, assess, analyze, and review the efficacy of our advertising, this Site's content, our Services and products, and other related assessments;
- Perform our obligations and enforce our rights; and
- Fulfill any other purpose for which you have consented and have provided information to us; and
- To keep our Services safe and secure, including through fraud monitoring and prevention, and investigate possible fraud or other violations of our Site’s service terms or this Privacy Policy and/or attempts to harm you.
For job applicants, we process your information to review your job application and for other employment-related purposes consistent with applicable law, including to contact you regarding your application and/or to contact you about other job openings that we believe may be of interest to you.
5. Disclosure And Exchange Of Information
We may disclose personal information for business and commercial purposes as described below. However, note that any mobile opt-in data and any consent obtained for our SMS messaging program will not be shared, sold, rented, leased, or otherwise transferred to any third party or affiliate for marketing or promotional purposes.
Service Providers. Affordable Care may disclose your personal information to service providers for a business purpose, which may include (but is not limited to) providing certain services for clients, tracking job applications, processing payments, and managing customer relations. When we disclose personal information for these reasons, we enter into a contract that describes the purpose and requires the recipient to both keep that personal information confidential and not use it for any purpose except for the purposes set forth in the contract.
The categories of service providers with whom we may share personal information include, but are not limited to, the following:
- Payment Processors;
- Finance & Accounting Tools;
- Product Engineering & Design Tools;
- User Account Registration & Authentication Services; and
- Website Hosting Service Providers.
Business Partners and Third Parties. We may share your information with our business partners and third parties to offer you certain products, or services. The categories of third parties or partners with whom we may share personal information include, but are not limited to, the following:
- Sales & Marketing Tools;
- Social Networks;
- Data Analytics Services; and
- Performance Monitoring Tools.
Legal and Similar Purposes. We may disclose your personal information for legal reasons. Specifically, we will share personal information with companies, organizations or individuals outside of Affordable Care if we have a good-faith belief that access, use, preservation, or disclosure of the information is reasonably necessary to:
- meet any applicable law, regulation, legal process or enforceable governmental request;
- enforce the Site’s service terms, including investigation of potential violations;
- detect, prevent, or otherwise address fraud, security or technical issues; or
- protect against harm to the rights, property or safety of Affordable Care, you, or the public as required or permitted by law.
We will attempt to notify you about legal demands for your personal information when appropriate in our judgment, unless prohibited by law or court order or when the request is an emergency. We may dispute such demands when we believe, in our discretion, that the requests are overbroad, vague or lack proper authority, but we do not promise to challenge every demand.
Merger, sale, or other asset transfers. We may disclose your personal information in the event of a business transfer. If Affordable Care establishes a new related entity, is acquired by or merged with another organization, or if substantially all of Affordable Care’s assets are transferred to another organization, personal information about Affordable Care’s users would typically considered a transferred business asset.
When you provide information to us or to our Affiliated Dental Practices, you may receive e-mail information, text messaging, phone call(s), or other forms of direct communications about our Services, with your consent where required. Additionally, we may release any provided information and/or data, to our Affiliated Dental Practices, or to a third party in accordance with our HIPAA Notice of Privacy Practices and this Privacy Policy.
Your provision of personal information is required in order to use certain parts of our Services. If you choose not to provide such personal information, you may not be able to access and use our Services.
6. Data Security
Affordable Care maintains reasonable administrative, technical and physical safeguards designed to protect personal information and other information against accidental, unlawful or unauthorized destruction, loss, alteration, access, disclosure or use. Although Affordable Care uses reasonable online security measures designed to protect your Personal Information, no data security measures are guaranteed to be completely effective. We are not responsible for any circumvention of security or privacy measures deployed on this Site.
Furthermore, it is common knowledge that transmission of information via the Internet is not wholly secure, and we cannot guarantee the security of your Personal Information transmitted to or through this Site or any of our Internet-based Services. Any transmission of Personal Information is at your own risk. By using our Site and Services, you acknowledge and accept these risks.
7. Links To Third-Party Sites
Our Service(s) may contain links to other sites that are not operated by us. If you click on a third-party link, you will be directed to that third party's site. Such links do not constitute an endorsement by us of those other websites, their content or services, or the persons or entities associated with those websites. This Privacy Policy does not apply to third-party websites. We have no control over, and assume no responsibility for, the content, privacy policies, or practices of any third-party sites or services. We encourage you to review the privacy policies and terms of all third-party websites or services that you may visit.
8. Retention
We will retain Personal Information for the period necessary to fulfill the purposes outlined in this Privacy Policy unless a longer retention period is required or permitted by law. After expiry of the applicable retention periods, we will either delete or anonymize such information. If there is any data that we are unable, for technical reasons, to delete entirely from our systems (for example, because your personal information has been stored in backup archives), we will put in place appropriate measures to prevent any further use of such data.
9. No Use By Children
We do not knowingly solicit data from or market to children under 16 years of age. By using our Services, you represent that you are at least 16. If we learn that personal information from users less than 16 years of age has been collected, we will deactivate the account and take reasonable measures to promptly delete such data from our records. We do not have actual knowledge about selling or sharing Personal Information of consumers under the age of 16. 10. Privacy Rights
Certain jurisdictions have specific legal requirements and grant privacy rights with respect to personal information, and we will comply with restrictions and any requests you submit as required by applicable law. If you are a California resident, see the “California Residents” section below for additional information as to your rights and to how to exercise those rights.
Nevada residents have the right to opt out of the sale of certain pieces of their information to third parties who will license or sell their information to others. However, Affordable Care does not sell your personal information as contemplated by Nevada law.
If you are a resident of one of the U.S. states listed below, you have one or more of the following rights: (1) to request additional information about our data collection, use, disclosure, and sales practices in connection with your personal information; (2) to request the specific personal information collected about you; (3) to request the deletion of the personal information we have about you, with exceptions; (4) to request a restriction on certain processing of personal information; (5) to request correction of inaccurate information; (6) to opt-out of the processing of personal information for the purpose of “profiling” in furtherance of decisions that produce legal or similarly significant effects; and (7) to opt-out of the “selling” and “sharing”/“targeted advertising” of personal information as such terms are defined under U.S. state privacy laws. Please note, Affordable Care does not sell personal information. Affordable Care does not share personal information/engage in “targeted advertising”. Affordable Care does not engage in profiling in furtherance of decisions that produce legal or similarly significant effects. To help protect your privacy and maintain security, we will take steps to verify your identity before granting you access to the information. We may need to request specific information from you to help us confirm your identity. This is a security measure to ensure that personal information is not disclosed to any person who has no right to receive it. When you make a request, we may also contact you to ask you for further information in relation to your request to speed up our response and require you to follow procedures so that we can verify your identity (and the applicable jurisdiction). The verification steps we take may differ depending on your jurisdiction and the request. Where possible, we will attempt to match the information that you provide in your request to information we already have on file to verify your identity. If we are able to verify your request, we will process it. If we cannot verify your request, we may ask you for additional information to help us do so.
We will respond to your request within the time period required by applicable law. However, we may not always be able or required to comply with your request, in whole or in part, and we will notify you in that event.
11. Jurisdiction-Specific Notices
Under the California Consumer Privacy Act, as amended by the California Privacy Rights Act (the “CCPA”), California residents have specific rights regarding their personal information. This section describes Californians’ rights and explains how California residents can exercise those rights.
Information Collected Related to California Residents.
During the last twelve (12) months, we have collected the following categories of personal information from consumers. We collect “sensitive personal information” (as defined in the CCPA) as described in this Privacy Policy.

Rights Specific to California Residents.
Under the CCPA, a California resident has the following rights: (1) to request additional information about our data collection, use, disclosure, and sales practices in connection with your personal information; (2) to request the specific personal information collected about you; (3) to request the deletion of the personal information we have about you; (4) to request a restriction on certain processing of personal information; (5) to request correction of inaccurate information; and (6) to opt-out of selling or sharing personal information. You may not be discriminated against for exercising your California privacy rights. You may submit a request to exercise your rights under the CCPA through one of two means:

We will endeavor to honor such requests unless such a request conflicts with certain lawful exemptions under California’s consumer privacy law. Please note that we are only required to honor requests to know twice in a 12-month period.
You may also designate an agent to exercise your privacy rights on your behalf. In order to designate an authorized agent to make a request on your behalf, you must provide a valid power of attorney, the requester’s valid government-issued identification, and the authorized agent’s valid government issued identification.
Upon receiving your request, we will confirm receipt of your request by sending you an email confirming receipt. To help protect your privacy and maintain security, we may take steps to verify your identity before granting you access to the information. In some instances, such as a request to delete personal information, we may first separately confirm that you would like for us to in fact delete your personal information before acting on your request.
We will aim to respond to your request as soon as reasonably practicable and consistent with any applicable laws. If we require more time, we will inform you of the reason and extension period in writing. If you have an account with us, we will deliver our written response to that account. If you do not have an account with us, we will deliver our written response by mail or electronically, at your option. In some cases our ability to uphold these rights for you may depend upon our obligations to process personal information for security, safety, fraud prevention reasons, compliance with regulatory or legal requirements, listed below, or because processing is necessary to deliver the Services you have requested. Where this is the case, we will inform you of specific details in response to your request.
In addition, California Civil Code Section 1798.83, California’s “Shine the Light” law, permits California residents to request and obtain a list of what Personal Information (if any) we disclosed to third parties for direct marketing purposes in the preceding calendar year and the names and addresses of those third parties. Requests may be made only once a year and are free of charge. Under Section 1798.83, California residents are entitled to request and obtain such information, by e-mailing a request to privacy@affordablecare.com.
12. Changes To This Privacy Policy
As iterated above, we will make changes to this Privacy Policy from time to time by posting an amended version of the Privacy Policy on our Site. It is our custom to provide notification on this page when we make material changes, which we shall do on the home page of the Site. The date that this Privacy Policy was last revised is identified at the top of the page.
You are responsible for ensuring that you periodically visit our Site and this Privacy Policy to check for any changes.
