The purpose of this document is to provide information related to communication between you, the patient (“you” or “your”), Affordable Care, LLCand ACI’s affiliates, subsidiaries, agents, and subcontractors ( “ACI”), and our independently-owned supported dental practices located throughout the United States (“Affiliated Dental Practices”) regarding the collection and use of your information and how we may communicate with you. This Communications Policy is effective when we contact you, you use our Services or whenever you provide your information to us, or our Affiliated Dental Practices, including but not limited to, when you provide your information in-person, online, over the phone, in a text message, in an e-mail, through a written letter, through use of our website, http://www.affordabledentures.com/ or otherwise by some other means or manner (“Provide Information”). You should read this Communications Policy carefully before you use our Services or before you Provide Information.
Information We Collect and How We Collect It
ACI and our Affiliated Dental Practices obtain information about you in order to conduct business, develop comprehensive statistics, serve you better, and for other business-related purposes. We collect information about users of our Site, as well as through any correspondence you provide to us whether through standard mail, e-mail, calls, or text messages. Account Status and Billing Communications
By signing this form, I agree to allow ACI and anyone who collects or communicates on ACI’s behalf to contact me about my account status, including past due or current charges, using prerecorded calls, text messages, email, and calls or messages delivered by an automatic telephone dialing system to any landline phone, wireless phone number, other contact number, or email address I have provided or that I obtain in the future. I further agree that ACI will treat any email address and phone numbers I provide as my private email or phone number that is not accessible by unauthorized third parties. Unless I notify ACI that my wireless service is based in a different time zone, calls will be made to my cellular device during permitted calling hours based upon the time zone affiliated with the mobile telephone number I provide.
Patient Authorization & Consent
I provide my express written consent to receive text messages, phone calls (including prerecorded or autodialed), and emails from Affordable Care, LLC., its Affiliated Dental Practices, agents, and contractors for informational, transactional, and marketing purposes. These communications may include appointment reminders, billing or insurance updates, treatment-related follow-ups, and occasional marketing messages about products, services, or promotions related to my care. Message frequency may vary, message and data rates may apply, and my consent is not a condition of receiving care.
